Introduction

The aim of this survey is to gain knowledge about current existing Long-Term Follow-Up (LTFU) care for Childhood, Adolescent and Young Adult (CAYA) cancer survivors within Europe. Furthermore, we aim to identify best practices and gaps in those areas. To get the most comprehensive picture possible, your experience is essential to us. 

Who can participate in the survey?

  • Health Care Professionals (HCP), who work/have worked in a follow-up care service for Childhood, Adolescent and/or Young Adult Cancer Survivors. 
How does it work?
  • To get the most comprehensive picture, we kindly ask you to complete the survey as a team, consisting of HCPs involved in follow-up care at your institution. 
  • It is possible to participate as a group of up to five persons: if possible with at least two different professions, to consider different perspectives. 
  • Within the project it is planned to develop a Europe-wide map of existing LTFU care and continuously expand it. You can agree or disagree at a later point if you wish for your LTFU care service to appear on the map.

Terminology
CAYA cancer survivor: C
hildhood, Adolescent or Young Adult cancer survivor (diagnosed before the age of 40)
LTFU care: Long-Term Follow-Up care for survivors which aims at monitoring for late effects of disease and/or treatment
LTFU care service: special clinic department or program for Long-Term Follow-Up care (not necessarily in a hospital setting)
HCP: Health Care Professional 

We are aware that terminology, recommendations and also the structures of follow-up care can vary from country to country. Therefore, when answering the following questions, please just think about the follow-up care provided in YOUR institution.

Your participation in the survey is entirely voluntary. Your IP address will not be collected. The survey will take about 15-20 minutes to fill out.
